Support and advice for those living with dementia, their carers or family members in Portsmouth.

What is it?

Remind is a service delivering support to those living with dementia and their carers throughout Portsmouth. Through practical help and guidance, we help you maintain independence and connect with your community.

Following a formal dementia diagnosis, the Remind team can set up an initial home assessment visit, where specialist workers will help you, your carer or loved one to create a plan with the right level of ongoing support.

Your plan may include:

  • Cognitive Stimulation Therapy (CST) which is an evidence-based therapy delivered in small groups to help memory, thinking skills, wellbeing and confidence.
  • Activity groups that give you the chance to meet others, share experiences and enjoy activities like arts and crafts, games and exercises to keep your mind engaged.
  • Practical home support to help you continue to live independently.
  • Help accessing local support and benefits you may be entitled to.

We fully recognise the vital role that carers play and we offer advice and emotional support. We'll always invite you to stay for a coffee and a chat whilst the person you care for attends the session.

Remind is delivered in partnership with Hampshire and Isle of Wight Healthcare NHS Foundation Trust.

Who is it for?

You will need to be formally diagnosed with Dementia or support some with dementia, and currently live in a PO1-PO6 postcode.

We welcome those of all ages, genders and backgrounds, as our support is personalised to you.

Information for professionals

NHS professionals can refer patients through the NHS System One.

Community organisations and professional carers can contact the service directly following a dementia diagnosis. A referral form must be completed and sent back to Remind via email or post.
