Support and advice for those living with dementia, their carers or family members in Portsmouth.

What is it?

Remind is a service delivering support to those living with dementia and their carers throughout Portsmouth. Through practical help and guidance, we help you maintain independence and connect with your community.

Following a formal dementia diagnosis, the Remind team can set up an initial home assessment visit, where specialist workers will help you, your carer or loved one to create a plan with the right level of ongoing support.

Your plan may include:

  • Cognitive Stimulation Therapy (CST) which is an evidence-based therapy delivered in small groups to help memory, thinking skills, wellbeing and confidence.
  • Activity groups that give you the chance to meet others, share experiences and enjoy activities like arts and crafts, games and exercises to keep your mind engaged.
  • Practical home support to help you continue to live independently.
  • Help accessing local support and benefits you may be entitled to.

We fully recognise the vital role that carers play and we offer advice and emotional support. We'll always invite you to stay for a coffee and a chat whilst the person you care for attends the session.

Remind is delivered in partnership with Solent NHS Trust.

Immediate changes to our Remind service

We regret to advise that our Remind service, run in partnership with Rowans Hospice to support those with Dementia and their families in the Portsmouth area, is changing. To ensure the long-term sustainability of The Rowans Hospice, the difficult decision has been made to discontinue support for this service. This will impact our NHS supported Remind Clinical Nurse Specialist Service, which will cease operation from 1 September 2024.

We understand that this change is unexpected for our existing service users. Solent Mind and Rowans Hospice are focused on working together to urgently inform those impacted by the changes and we will continue to support our families as much as possible and assist with future planning.

Where Solent Mind are unable to support due to the removal of Clinical Nurse Specialists, service users will be directed to specific NHS services designed to support those experiencing dementia either through their GP, NHS 111 or Older Person’s Mental Health (OPMH). We will continue to advocate for our service users on an individual basis to ensure that they get the support they need.

Here are the key services available:

  • Rowans Living Well services will continue to support carers. This is a self-referral service and patients and carers will be supported with scheduled group activities, as well as signposting
  • Solent Mind Remind service will provide support with social stimulation, dementia education, safety at home and help with connecting to other organisations for additional support as needed. Contact: 023 9421 7404
  • Carers Centre - providing practical and emotional support for carers and assistance in finding respite options. Contact: 023 9285 1864
  • Older People Mental Health Team - access via referrals from health care professionals only. Please speak to your GP. Contact: 0300 123 3923
  • Portsmouth Adult Social Care - can support with personalised care plans and assistance with daily activities. Contact the Help Desk: 023 9268 0810
  • Medical assistance - Please contact your GP if you have any medical concerns or access the out-of-hours GP service by calling 111. If you or someone you care for is seriously ill or injured call 999

Who is it for?

You will need to be formally diagnosed with Dementia or support some with dementia, and currently live in a PO1-PO6 postcode.

We welcome those of all ages, genders and backgrounds, as our support is personalised to you.

Information for professionals

NHS professionals can refer patients through the NHS System One.

Community organisations and professional carers can contact the service directly following a dementia diagnosis. A referral form must be completed and sent back to Remind via email or post.
