Remind is a service delivering support to those living with dementia and their carers throughout Portsmouth. Through practical help and guidance, we help you maintain independence and connect with your community.
Following a formal dementia diagnosis, the Remind team can set up an initial home assessment visit, where specialist workers will help you, your carer or loved one to create a plan with the right level of ongoing support.

We fully recognise the vital role that carers play and we offer advice and emotional support. We'll always invite you to stay for a coffee and a chat whilst the person you care for attends the session.
Remind is delivered in partnership with Solent NHS Trust.
We regret to advise that our Remind service, run in partnership with Rowans Hospice to support those with Dementia and their families in the Portsmouth area, is changing. To ensure the long-term sustainability of The Rowans Hospice, the difficult decision has been made to discontinue support for this service. This will impact our NHS supported Remind Clinical Nurse Specialist Service, which will cease operation from 1 September 2024.
We understand that this change is unexpected for our existing service users. Solent Mind and Rowans Hospice are focused on working together to urgently inform those impacted by the changes and we will continue to support our families as much as possible and assist with future planning.
Where Solent Mind are unable to support due to the removal of Clinical Nurse Specialists, service users will be directed to specific NHS services designed to support those experiencing dementia either through their GP, NHS 111 or Older Person’s Mental Health (OPMH). We will continue to advocate for our service users on an individual basis to ensure that they get the support they need.

NHS professionals can refer patients through the NHS System One.
Community organisations and professional carers can contact the service directly following a dementia diagnosis. A referral form must be completed and sent back to Remind via email or post.
