Remind, Dementia Support in Portsmouth

 Living with Dementia

We at Remind understand that this can be an uncertain and challenging time for those living with dementia, their carer and family members.

We are here to help and support you through your journey.

The Remind team delivers a high quality advice and support service to dementia patients and their carers across the Portsmouth area. Our team consists of highly motivated, compassionate professionals who are ready and willing to support you and people who care for you.

 

How can we help?

Following a diagnosis of dementia by your GP, we will arrange to meet with you either during one of our appointment sessions or we will arrange a more in depth meeting to complete an initial assessment. The initial assessment will be in the comfort of your own home, with a Remind staff member, your carer and yourself. The aim of the meeting is to assess and discuss the needs of the client with dementia and also to advise the carer of how we can provide support. We will take care to listen to you, so that we fully understand your current needs and be in a position to tailor our support accordingly.

Together, we will agree on a plan that is tailored to you.  Every individual case is different and we will work with you to agree the right level of support going forward.

  • The Remind team includes Intensive Engagement Support Workers, Rowans Clinical Nurse Specialists (Dementia) and an Activity Group Co-Ordinator. We work closely with the NHS Older Persons Mental Health Team (OPMH) at St Mary’s Hospital, Portsmouth; GPs and local hospitals.
  • Remind facilitates Cognitive Stimulation Therapy (CST) http://www.cstdementia.com/ which is an evidence-based therapy that can help memory, thinking skills, wellbeing and confidence.  It is delivered in small groups with carers invited to stay for a coffee whilst the person with dementia attends the session. Attendance is subject to an assessment.
  • Activity Groups – These groups give you the chance to meet others, share experiences and enjoy a range of activities including exercises to keep your mind engaged. We also focus on fun activities like arts and crafts and games in a friendly and supported way. Remind groups are extremely popular and have proven to be beneficial during your dementia journey.
  • Remind can help you with practical things, from preparing your home to best suit you to continue to live independently, to advising you what benefits you may be entitled to.

 Above all, we look to support you to assist in maintaining your independence.
