Storing and Recording of Information
We keep a record of your details, and notes from your appointments and attendance on our patient management system which is completely secure, confidential within Solent Mind and complies with Data Protection legislation. We keep the information to book appointments, keep track of progress and to record things pertinent to your care; similar to the way GPs and other healthcare services keep information. Anonymized information is also used to report service compliance to our commissioners and funding bodies and to help us track trends and improve how we work. If you have any concerns then you can discuss these with your individual Solent Mind worker.
Sharing of Information
When start receiving support from one of our teams we’ll ask for your permission to keep other agencies involved in your care informed of your progress with us. The reason for this is to maintain our duty of care to you. GPs, community and hospital based care teams are crucial to patient care and it is very important they are aware of the treatment that you may be receiving through us. It also means we can often work together to give you the best help. If you have any concerns the information we share then you can discuss this with your individual Solent Mind worker.
Requesting information held about you
You have a legal right to access personal information held about you by an organisation. This right is protected by the Data Protection Act 1998. To access your personal information, you need to make a request in writing, or talk to your Solent Mind worker.
